The District of Columbia Developmental Disabilities Fatality Review Committee (DDFRC) is pleased to present its thirteenth Annual Report. DDFRC was initially established in February 2001, by Mayor's Order 2001-27, and re-established in September of 2009 by Mayor's Order 2009-225 as the Developmental Disabilities Fatality Review Committee (see Appendix A). The Committee is charged with examining the events surrounding the deaths of individuals 18 years of age and older who were receiving services from the District of Columbia Department on Disabilities Services (DDS) at the time of death.
